New Electronic Indemnity Form

For the convenience of participants taking part in Dragon Boating,  Kayaking and Canoeing, and Surfing activities at Singapore Sports Hub, we are replacing hard copy indemnity forms with electronic indemnity forms. This comes into effect from 2 May 2025.

The electronic indemnity declaration has a validity of one year and covers participation across the above listed activities. Fill in your electronic indemnity form here: https://indemnityform.sportshub.com.sg/.
